Your labs are fine. So why are you still exhausted?

A QUESTION WE HEAR MOST WEEKS

"My bloodwork came back normal, but I'm exhausted all the time. What am I missing?"

This is one of the most common things we hear in an intake, and it comes with a very specific kind of frustration. You did everything right. You saw your doctor, you ran the tests, and they came back clean. And you are still flat by mid-afternoon.

Here is the part almost nobody explains. "Normal" on a standard panel means you do not have a diagnosable disease. It does not mean your body is running well. Those panels were built to catch anemia, thyroid disease, and diabetes. The low-grade dysfunction that leaves you tired, foggy, and slow to recover sits just under the threshold of a flagged result, and it was never what those tests were designed to find. There is a real gap between "not sick" and "actually functioning," and most people get told they are fine when they are sitting right in the middle of it.


WHAT WE ACTUALLY LOOK AT

When someone brings us this picture, the first thing we do is look again at what was measured, and at what was left off. Ferritin is the one I come back to most. A result of 18 can sit inside the "normal" range and still be the reason someone is exhausted, because ferritin reflects your iron stores, and iron drives your energy system long before a shortage ever shows up as anemia. After that we look at free T3 next to TSH, your cortisol pattern across the whole day rather than a single morning reading, fasting insulin, and hs-CRP for inflammation. Then we look at recovery you can actually measure: sleep stages and heart rate variability tracked over weeks, not one restless night.

ONE STUDY WORTH KNOWING

This is not only a clinical hunch. A double-blind, placebo-controlled trial published in the BMJ took 144 non-anemic women with unexplained fatigue and gave half of them iron and half a placebo for four weeks. The women on iron reported meaningfully less fatigue, and the benefit was concentrated in those with low or borderline ferritin, none of whom were anemic. A later randomized trial in the CMAJ found much the same thing.

Two honest caveats, because they matter. These were small, short trials in menstruating women, so this is not a universal rule for everyone who is tired. And iron is not a supplement to start on your own, because too much of it causes its own problems. But the signal is clean and it is the whole point of this email: you can be "not anemic" and still be low enough on iron to feel it every single afternoon. The standard panel simply was not looking.

WHAT YOU CAN DO NEXT

  • Ask for the fuller picture, not just the standard panel: ferritin (not only hemoglobin), free T3 alongside TSH, fasting insulin, hs-CRP, and a vitamin D level. Five additions that change what you can actually see.
  • Keep a simple energy log for two weeks: time of day, how you slept the night before, what you ate, what was stressful. The pattern usually points at the mechanism faster than any single test does.
  • Get one objective recovery number. A basic wearable tracking HRV and sleep stages will tell you more about how you are really doing in a week than years of guessing.
